The underhand grip tricep press-down is a variation of the classic tricep press-down move that gives you a more favorable position to target the long head of the triceps.
The long head of the triceps is the largest part of your arm and is the thick slab of muscle in the back of your arm. Building this muscle gives you wider looking arms from behind, and will help your arms look much bigger in general.
